Q2:Helmet, Safety shoes and Special Vest.-If required, wear PPEs during work.安全头盔，安全鞋，反光马甲,长袖工作服。-如需要，作业时佩戴个人防护用品。

Q3:There is work permit on site with authorization.现场有作业许可证（已正确填写并授权操作）。

Q4:Barrier tape and safety warning to mark the area.警示带和警示语，用以区分工作区域。

Q5:Take actions to protect the floor when required.采取措施保护地面。

Q6:The main responsible person from supplier should be present on site to supervise the process. -EHS from supplier as well if required.供应商负责人应在现场进行监工。如需要，供应商EHS人员同时应在现场监工。

Q7:吊装作业，Warning signs of lifting with limited access.-There is no person allowed into the marking area during lifting.吊装区域警示标志，授权进入。-吊装过程中，不允许有任何人员在吊颈范围内。

Q8:Signalman location is clearly visible by crane operator.吊装指挥站在司机可见范围内。

Q9:Affected area is cleared of all obstructions that may hinder lifting works. 清除吊装区域所有障碍物。

Q10:Outriggers of crane fully extended.吊车支脚完全撑开。Outriggers of crane rested on steel plate. 吊车支脚安放在撑脚板上。

Q11:Lifting hooks provided with safety latches.吊钩上有安全锁扣。

Q12:Load is properly and securely rigged in a right way.吊装物按照正确方式有效绑扎。

Q13:高空作业：A barricade or fence should be set up under working at height area. 高空作业区域的下方应有警示标记或围栏。

Q14:The scaffold shall only be used when the appropriate Scaffold Inspection Tag is attached. Forbid use installing or dismantling scaffold.脚手架只有经过检查并贴上合格标签后才能使用，禁止使用正在搭建或拆除的脚手架。

Q15:Use mobile scaffold on paved and level surface only; 在平滑地面上使用移动脚手架。Lock mobile scaffold wheels before use;br使用移动脚手架前把轮子锁死。Disembark and remove tools and materials from mobile scaffold before moving it to new location.推动移动脚手架前应确保员工、工具和材料都不在脚手架上。

Q16:最高倾斜度不能超过1:4比率。Worker to support base of ladder or secure top of ladder with rope;使用时扶着梯子的下端或稳固梯子上端。 Only one person is to ascend or descend a ladder at a time;梯子上只能站一个人。Do not move the ladder when people stand on ladder在有人站在梯子上时，不能移动梯子。

Q17:Prevent measure is ok, such as guard rails, toe board, barricade.防护措施到位，如护栏、踢脚板、围栏等。

Q18:Appropriately use safety harness,br正确使用安全带：brThere is anchor point, such as lifeline.br现场有系挂点，如生命线等。brMust be high enough to prevent falling worker from hitting lower level structure;br安全带必须位于足够高度从而避免工人在坠落时撞击低层结构。brDo not uses snap hook for to wing, hanging or lifting purposes, required 100% Tie-off.br不要将弹簧钩作为牵引，悬挂或吊装用途；如有移动请交替系挂安全带，以保证100%系挂br

Q19:动火作业 ：There are at least two qualified extinguishers on site.现场放置至少2个合格的灭火器。

Q20:There are no combustible materials, such as wood, plastics, cardboard, etc.现场无易燃物品，如木头、塑料、纸板等。

Q21:There should have special actions for trench or others within 10m.距动火点10m以内对地沟等进行保护。

Q22:临时用电：When use the equipment by electrical cabinet ,must confirm by facility department当使用设备或配电柜电，必须经过厂务部门确认
